Short answer

A complete campaign achievement guide for Call of Duty: Modern Warfare 4, covering all 18 achievements. The guide goes mission by mission. Each mission has its achievements, short step-by-step instructions, and Realism tips, so you can find everything for the mission you're playing in one place…

Steam Community guide by XVIII.

General Information

Two achievements are difficulty related, so you need 2 playthroughs to get 100%:

  • Hold the Line: Finish the game on any difficulty.
  • Was it Worth it?: Finish the game on Realism.
    Playthrough 1: Recruit
  • Get all achievements in this run.
  • Play on Recruit, and switch the difficulty for the achievements that need a higher one: Safety's On and Revenge (Hardened or harder), and Can't Touch This (Veteran or higher). The guide tells you when to switch.
  • Finishing the game unlocks Hold the Line and Realism difficulty, which is locked at first.
    Playthrough 2: Realism
  • Finish the game on Realism for Was it Worth it?.
  • Since you already have everything else, this run is only about getting through the game.

General Realism Tips

Mission-specific Realism tips are in each mission's section. These apply to the whole playthrough:
Important: Play the missions in order. If you play the last mission, Greater Good, on Realism and then go back to earlier missions, Was it Worth it? won't unlock until you play Greater Good again in order.

  • Always keep your plates up.
  • There are a lot of ammo boxes and armor plates as you progress, so grab them.
  • Distance is key in most missions. Keep a good distance whenever you can.
  • Stay behind cover and peek by mounting your weapon whenever you can.
  • Pick weapons with a high magazine size, since that means more reserve ammo.

1. Mayhem

IconAchievementDescription

Skeet ShootingKill 10 paratroopers in 'Mayhem' before they hit the ground.

Skeet Shooting

You don't need to finish the mission. It unlocks as soon as you kill your tenth paratrooper in the air.

  • Reach the stadium field with your squad.
  • Shoot the first wave directly above you while they're still in the air.
  • Ignore the ones that have already landed and move on to the next wave.
  • Keep going until you reach 10.Tips
  • Lower your FOV to 60. It makes them much easier to hit.
  • If it didn't pop, restart the checkpoint and try again.

Realism Tips

  • When you get to the point where you push the fuel truck, stay back and clear the enemies slowly. This is a choke point and you can easily be killed.
  • Once the enemies are clear, dash to the crate with the RPG and use it to shoot the truck.

2. Out in the Cold

IconAchievementDescription

Safety's OnDefeat all enemies in the kitchen without firing your gun on Hardened or harder.

Can't Touch ThisDefeat Makarov on Veteran difficulty or higher without getting hit in the alley.

Safety's On

Take down all 5 enemies in the kitchen without firing a single shot. Needs Hardened or higher.

  • Load into the mission and change the difficulty to Hardened or Veteran.
  • After the elevator, grab the bottles from the bar table stands before the kitchen area.
  • Behind the kitchen, bash through the door and run at the enemy on your right. Hit him with a bottle and do a takedown.
  • Repeat on the 2 other enemies close to you.
  • Dash to the last two behind the counter and punch them.Tips
  • Switch to melee before going in. Takedowns while holding a weapon execute with the weapon and don't count.
  • If you fire your gun, restart the checkpoint.

Can't Touch This

Beat Makarov without taking a single hit. Needs Veteran or higher.

  • Switch to Veteran when you engage Makarov on the rooftop.
  • The fight has two parts. In part 1, hold guard/parry and click attack while holding it.
  • In part 2, Makarov attacks faster. Holding guard too long stuns you, so attack back in between.
  • Don't attack quickly while his guard is up. He'll hit you back.Tips
  • A red indicator on your HUD means you took damage. Restart the checkpoint when you see it.

Realism Tips

  • In the kitchen section, open the door slightly and peek through from safety.
  • The Makarov fist fight plays the same as the achievement, but you can get hit. Realism is just Veteran with a minimal HUD.

3. Following Orders

IconAchievementDescription

No Harm DoneObtain the defector's location in 'Following Orders' without hurting any civilians.

No Stone UnturnedConfiscate all of the contraband and read all of the notes in 'Following Orders'.

No Harm Done / No Stone Unturned

Confiscate all of the contraband and read all of the notes and Obtain the defector's location in 'Following Orders' without hurting any civilians.
Start by collecting all the contraband in the first house. During this you'll talk to the woman, so choose the dialogue options that send her to join her husband in the hallway. After that, you can speak to the man and question him about each contraband item one at a time.
Don't hit or kill either civilian at any point. Once you get the defector's location without harming anyone, the achievement unlocks.
First house

  • Note From Your Friend and Heart Medicine: Go through the door on the right and search the cupboard. Inside is a red box with a number lock. The code is 214, or you can shoot the lock off.
  • MP3 Player (contraband): Take the first door on your left after the main entrance and open the cabinet.
  • Western Novel (contraband): Go through the second door on your left and search the cabinets.
  • Baekseolgi-Tteok Recipe (note): Head to the kitchen, where the second civilian is, and check the cupboard on your left. This note also gives you the red toolbox code.

    Third house
  • Note From Your Neighbor: Enter through the first door on the left, then take the second door on the right into the bedroom. The note is in the cabinet.
  • Note From Mom: Go through the sliding doors past the bedrooms, take the first door on the right, and pick up the key from the cupboard by the window. Then return to the living room and head to the kitchen, where the kettle is boiling. Open the hatch to the hideout and bring the civilian out by capturing her twice, talking to her, or threatening her. Once she leaves, take Note From Mom from the hideout. Keep in mind you don't necessarily need the key to get the note, you can just call the civilian out when you are on top of the trap door.

Realism Tips

  • Simply proceed through the mission as usual. No combat is required.

4. Hired Gun

IconAchievementDescription

Bullet TrainKill 50 enemies while standing on train cars in 'Hired Gun'.

Bullet Train

Kill 50 enemies while standing on train cars in 'Hired Gun'.
You can get kills for this from the start of the mission all the way to the final part, where you shoot down the helicopter and fight Graves. Only kills made while you're standing on a train car count, so ground kills don't.

  • Flatbed cars, roofless boxcars, and coal or aggregate hopper cars all work.
  • Moving and stationary cars both count. Anything on the rails works.Tips
  • Play on Recruit. Regular is unpredictable. Some players get it in one run, while others shoot every enemy and still end up short of 50.
  • Use the train cars as cover while you shoot.
  • If you reach the end short of 50, replay the mission or reload a checkpoint and keep going.

Realism Tips

  • Before engaging the helicopter, stay in cover and eliminate nearby enemies. Use the grenade launcher to destroy incoming vehicles.
  • Valeria carries extra grenade launcher ammo, so grab it when needed.
  • Once the helicopter is down, approach Graves and aim for headshots. Watch out for his Semtex grenades, as they are the most dangerous part of the fight.

5. Losing Ground

IconAchievementDescription

Raining ThunderKill 15 enemies with 3 mortar rounds in a row in 'Losing Ground'.

Raining Thunder

Kill 15 enemies with 3 mortar rounds in a row in 'Losing Ground'.
The game doesn't limit you to only 3 shots. What counts is the combined kills from any 3 mortar shots in a row, so you can keep firing until you hit 15.

  • Once you are at the mortar, wait for the large wave coming from the right side. This is the best time to do it.
  • Fire at the groups of enemies and aim for about 5 kills per shot.
  • If you don't reach 15 kills across 3 shots in a row, reload the checkpoint and try again. You also might need to restart the mission if you fail to get the 15 kills.Tips
  • Aim at groups instead of single soldiers.
  • Don't waste a shot on an area with few or no enemies.

Realism Tips

  • When you reach the mortar section, keep spamming mortar strikes to clear enemies quickly, but be mindful of civilians crossing the area.

6. Traffic

Realism Tips

  • During the first car chase section, your armor is limited, so use it carefully.
  • After the second car chase, once you're on foot, grab a Riot Shield from the enemies. It will help a lot in the sections ahead.
  • Blue vans contain armor and ammo, so use them to restock when needed.
  • In the final section, use C4 with the Riot Shield to eliminate enemies safely, especially the armored trucks. Be mindful of civilian vehicles on the road.

7. Gone Dark

Realism Tips

  • Simply proceed through the mission as usual.
  • During the fist fight with Ghost, use the same strategy as you did against Makarov.

8. Assassins

IconAchievementDescription

RevengeKill everyone after Shin dies in 'Assassins' on Hardened or harder difficulty.

PacifistComplete 'Assassins' by only killing one enemy.
These two conflict, so get them in separate attempts.

Revenge

Kill every enemy after Shin dies, on Hardened or higher. It unlocks as soon as the last enemy dies, so you don't need to finish the mission.

  • Set the difficulty to Hardened or higher before starting the mission.
  • After Shin dies, kill every enemy in each area. Don't move on until the area is clear.
  • In the garden, watch for enemies up the stairs on your left as you enter.
  • When you open the bunker door, more enemies spawn behind you.Kill the 13 enemies in the library and kitchen before you reach the garden.
    Tips
  • Use your knife and throwing knives to stay stealthy. You have no armor, so you can die easily on Hardened.
  • Double check each area before you reach a checkpoint, since there are a lot of enemies before the bunker.

Pacifist

Finish the mission with just one kill. The game forces one kill during a cutscene, and that counts as your one.

  • In the kitchen area, throw bottles to distract enemies.
  • You can also hide inside the storages in the kitchen area.

Realism Tips

  • Avoid combat as much as you can, the same way you got Pacifist.
  • Use throwing knives to get through the kitchen area safely.
  • Make sure to kill the sniper in the middle of the garden.

9. Survivors

Realism Tips

  • Simply proceed through the mission as usual. No combat is required.

10. Stranded

IconAchievementDescription

**Incoming!**Destroy every vehicle reinforcement at the SAM Site before they unload in 'Stranded'.

Time to CrateOpen every Yellow Supply Crate in 'Stranded'.

ScavengerRepair everything in Base Camp.

NoteworthyRead 50 different Notes.

Incoming!

Destroy every vehicle reinforcement at the SAM Site before they unload in 'Stranded'.
Use explosive drones to destroy every vehicle before the soldiers get out. The achievement pops once all the vehicles and the BMP are destroyed.
Important: Restarting from a checkpoint made this achievement fail for me, so try to get it on your first attempt and avoid restarting.

  • Do the jamming station mission first so you unlock explosive drones.
  • Stock up on about 10 drones, just in case. You can grab more from the boxes around the statue.
  • When you first get to the mission, there's a group of enemies exercising, and a vehicle will pass near them. Use explosive drones to hit both the group and the vehicle, just to be sure.
  • Go to the SAM Site and look north. You'll easily see the vehicles coming down the main road.
  • Fly the drones at the vehicles as they approach. Don't wait for them to arrive.Tips
  • Ignore the "out of range" warning on the drone. The range is very generous, so you can still hit the vehicles.
  • Don't use the craftable anti-material sniper rifle. The soldiers get out of the vehicle as soon as you hit it.

Time to Crate / Noteworthy

Open every Yellow Supply Crate and read 50 different notes in 'Stranded'.
You'll get every crate relatively fast if you check every outpost and look through the points of interest (POIs). Most crates are marked on the map, but a few are hidden. Use the Supply Tracker tactical item to find those.
The open world mission Stranded has 58 notes in total. You only need 50 different ones, so you can miss a few.
Hidden bunkers
There are two hidden bunkers, and one of them holds a yellow crate.

  • NW Island Hidden Bunker at (615, 169), password 0008. Has a yellow crate inside.
  • Farm Fields Hidden Bunker at (631, 128), password 0330. Has supplies inside, but no yellow crate.
    Tips
  • If you're missing one crate, check the moving train, the patrolling truck in the snowy area to the northeast, and the container at the artillery position that's blocked by breakable crates.
  • The Supply Tracker readings move closer and farther if the crate is on a moving vehicle.
  • Interacting with the craftables inside the Base Camp will also count towards your total notes prior to crafting them.

Scavenger

Repair everything in Base Camp.
Base Camp has a lot of gear you can repair, and the achievement needs every repairable item done. It looks bugged for a lot of people, but in most cases they just missed a craftable or upgrade that only appears after you pick up something out in the world.
You'll need a good stock of materials for this. Explore the map and collect yellow crates and outposts before heading back to base.
The two things people miss often

  • Semtex: This is the lethal you can craft at the base. If you've already picked up one of the lethals you could have crafted, it won't show up. Throw away all your lethals, then go back into the base and check if you can craft it.
  • Vulcan sniper rifle suppressor: At the SAM Site objective in the top left of the map, there's a Vulcan sniper rifle in front of the statue. Picking it up adds a craftable suppressor on the table under the ammo bag. Several players had every other repair done and got the achievement only after this.
    Tips
  • If you did everything on the list and nothing unlocked, it's very likely the Vulcan suppressor or the Semtex.
  • You don't have to get this on your first run. The game keeps your mission progress, so you can come back after finishing the campaign, check the base, and it will unlock once everything is repaired.

Realism Tips

  • Search the area until you have enough supplies to craft the Silenced Assault Rifle, then proceed with the missions.
  • Grab a Silenced Sniper Rifle from enemies or craft one at the base.
  • Leave the SAM Site for last, so you have explosive drones to make it easier.
  • Avoid unnecessary combat. I cannot stress this enough: fighting head-on is nearly impossible without an armor plate carrier, so prioritize stealth whenever possible.

11. Defection

Realism Tips

  • Progress through the first section as usual.
  • Once outside the hangar, quickly collect all the explosive drones from the two nearby crates.
  • Immediately use the drones to eliminate large groups of enemies.
  • After taking out the mortars with Dunn, switch back to Jae and continue fighting until the game forces you to switch back to Park.
  • Keep switching between characters whenever you take damage to recover health and continue progressing through the objective.
  • When the extraction helicopter arrives, wait a bit before moving toward the extraction point. Take your time and slowly eliminate the enemies along the way.

12. Counter Attack

IconAchievementDescription

Long ShotSnipe all 4 enemies firing the mortars in 'Counter Attack'.

Long Shot

Snipe all 4 enemies firing the mortars in 'Counter Attack'.
Progress through 'Counter Attack' until you reach the roof of the hotel. Two mortar teams spawn as you move through the objective, with 2 soldiers each. Kill all 4 with the sniper rifle.

  • The first mortar team spawns on the left side of the city hall.
  • The second mortar team spawns a bit further left, on top of the garage roof.
  • Once you kill one soldier, the other one at that mortar runs away.
  • If you miss and one escapes, restart from the checkpoint.

Realism Tips

  • Friendly APCs have armor and ammo behind them, so make sure to restock whenever needed.
  • Use your targeting device as often as possible to take out groups of enemies efficiently.
  • On the hotel rooftop, use the missile launcher whenever it's ready.

13. Escape

IconAchievementDescription

Never ForgottenRecover the Dog Tags in 'Escape'.
Recover the Dog Tags in 'Escape'.
Gaz's Dog Tags are locked inside a wall safe, and you need to find the code first.

  • At the start of the mission, find the Confiscation Protocol Reminder note on a desk in the office across from Cell 41.
  • Go past the guard checkpoint into the security office and find the note with the updated code: 8410.
  • Progress through the mission, rescue Ghost, and continue ahead with him.
  • Clear
